Job Description: Flex Team Specialist (LTSS Service Care Manager)

Shift: Monday to Friday, 8 AM–5 PM, with potential overtime.

Location: Field-based with up to 2 hours of travel one way; remote documentation on non-travel days.

Duration: Potential for extension.

Position Overview: The LTSS Service Care Manager will conduct member assessments, document findings, and determine service authorizations for personal attendant services. This role involves 75% travel for in-home member visits (3 days per week) and 2 remote workdays for documentation and follow-ups.

Responsibilities:

  • Assess members' needs for personal attendant services and complete the H2060 form.
  • Document and submit assessments for authorization/denial within three business days.
  • Evaluate complex/high-risk members and develop care plans for optimal outcomes.
  • Advocate for members, liaising with families, physicians, and community resources.
  • Educate members/caregivers on preventive care, treatment options, and healthcare benefits.
  • Coordinate and authorize services such as meals, transportation, and daily living assistance.
  • Maintain accurate documentation in the care management system.
  • Ensure compliance with clinical, state, and federal guidelines.
  • Provide feedback for improving quality of care and cost-effective service delivery.

Performance Expectations:

  • Achieve a quality assessment standard of 92% or higher.
  • Complete documentation within a three-business-day turnaround.

Qualifications:

  • Required:
  • 5+ years of experience working with aged, blind, or disabled populations.
  • Bachelor's degree or LVN with 2–4 years of related experience.
  • Proficiency in Microsoft Windows applications.
  • Preferred:
  • Background in social work, care management, or nursing.
  • Bilingual candidates strongly preferred.

Interview Process:

  • Method: Microsoft Teams or Zoom
  • Number of Interviews: One.

Onboarding:

  • Remote login and IT setup on the first day.
  • Virtual training via Teams and meetings with leadership for orientation.

Compensation: $26.50/hour.

Work Environment: Field-based role with a remote work component. Regular travel is required. This is an excellent opportunity for experienced service coordinators, case managers, LVNs, or social workers passionate about serving high-risk members in need of long-term care services.
